Job Description Job Description Job Title: RN – Home Health Location: Tulsa, OK (Travel to patient homes in Skiatook & surrounding areas) Schedule: Monday–Friday | 8:00 AM – 5:00 PM | Day Shift Incentives: $10,000 Sign-On Bonus (2-year commitment) | Mileage reimbursement at $0.70/mile Job Summary The Home Health Registered Nurse provides comprehensive in-home nursing care by assessing patient needs, developing individualised plans of care, coordinating services, managing medications, and educating patients and caregivers to support safe, effective recovery and timely discharge. Key Responsibilities Assess patient eligibility and care needs for home health services Develop, implement, and revise individualised, patient-centered plans of care Provide skilled nursing care per physician orders and agency protocols Perform medication reconciliation and communicate issues to the care team Coordinate care with physicians, therapists, aides, and interdisciplinary teams Supervise Home Health Aides and complete required supervisory visits Educate patients and caregivers on disease management, safety, and emergency care Document all care accurately in the EMR and meet productivity standards Ensure compliance with infection control, QA, QAPI, and PDGM requirements Requirements Active Oklahoma or Multistate RN License Minimum 1 year of home health or related nursing experience (required) Valid driver’s license, reliable vehicle, and acceptable driving record Strong clinical, communication, and computer skills Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
