Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 02/02/2026 - Duration: 52 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Name: Yale Campus - Warren Clinic Pay Range:$33.15 - $46.27 Shift/Schedule Details: Days / Mon - Fri / 8:00am - 5:00pm Incentives (Sign-on Bonus / Relocation): 10 K Sign On with 2 year commitment Additional Notes (if any): Locals preferred. Must have Home Health experience and be able to meet patients at all levels. Valid driver’s license, good driving record, and own vehicle required. Must carry minimum liability coverage; fingerprinting required. Flexible scheduling to accommodate patient needs.
